Viagra in Zaragoza when you smoke: why your GP will check your heart first

Smoking plus erection trouble is a reason to have your heart and blood vessels checked, not just a reason to buy a pill. Nitrates rule out every one of these tablets. In Spain, prescription-only medicines may not be sold online, and customs can hold a parcel that contains them. We do not check prescriptions: talk to your doctor before you order.

When erections start to let you down, the obvious move is to reach for a pill. If you smoke, a more useful question is why the erection is failing in the first place. The European Association of Urology (EAU) lists cigarette smoking among the vascular causes of erectile dysfunction, and its guideline calls ED an established early sign of atherosclerosis that predicts heart and circulation problems in its own right.

This page is for English speakers who live in Zaragoza. It covers what a sensible check-up looks like, how to get help to quit through the Aragonese public health service, what the medicines are called here, and what Viagra or Cialis can realistically do while you deal with the cigarettes.

Small arteries, early warning

An erection is mostly plumbing. The inner lining of the penile arteries releases nitric oxide, the vessels widen and blood fills the erectile tissue. Tobacco smoke injures that lining and speeds up atherosclerosis, the same furring-up that eventually narrows the coronary arteries.

A common explanation for why the penis tends to show it first is size: its arteries are narrower than the heart's, so the same amount of damage makes itself felt sooner. Whatever the precise mechanism, the guideline draws a practical conclusion: any man with ED should have his cardiovascular risk assessed.

What a check-up in Zaragoza usually involves

  1. Questions about how and when the problem started, whether you still wake with erections, your sex drive, and how much you smoke. Expect your smoking to be recorded in pack-years: the calculator further down shows how.
  2. Blood pressure, weight and a physical examination.
  3. Blood tests for glucose, cholesterol and other lipids, and total testosterone. That is the basic work-up the EAU recommends for every man with ED.
  4. An estimate of your overall cardiovascular risk. If it lands in the middle band, further heart tests may be suggested.

Bring a list of everything you take, including over-the-counter products and anything brought from home, with the active ingredient written next to each brand. Spanish pharmacists and doctors work with generic names.

Doctors, hospitals and duty pharmacies in Zaragoza

Registered with the public system. If the Spanish Social Security institute (INSS) recognises your right to public healthcare, as a worker, pensioner or EU citizen with an S1, Aragon's health service, known as the Salud, issues you a tarjeta sanitaria (health card) and assigns you a GP at your local centro de salud.

Here for a limited time. Visitors can use a European Health Insurance Card (EHIC; TSE in Spanish) for care that becomes necessary while they are here. It is not meant for planned treatment.

Private insurance. Prescriptions written by private doctors practising in Spain are accepted by every pharmacy in the country. Spanish pharmacies also accept prescriptions issued in other EU countries. They are not obliged to accept a UK prescription, though some pharmacists may, at their discretion; a US one isn't covered either.

Whatever your route, ED tablets are not funded by the public system: you pay the full price. If you need a specialist, your GP refers you to urology. The two big public hospitals both have urology departments covering men's sexual health: Hospital Universitario Miguel Servet, which has an andrology unit, and Hospital Clínico Universitario Lozano Blesa. Which one you see depends on where you live.

Out of hours you need the farmacia de guardia, the duty pharmacy on the rota. The Zaragoza pharmacists' college links to the province's duty pharmacy search from its website, in Spanish.

Ring 112 straight away for chest pain, fainting or near-fainting, or a sudden loss of sight or hearing after one of these tablets, and for an erection that hasn't gone down after 4 hours. Overdose worries go to Spain's poisons line, the Servicio de Información Toxicológica, on 91 562 04 20.

Viagra and Cialis for a smoker: realistic expectations

Sildenafil, tadalafil and vardenafil boost the nitric oxide signal that smoking wears down. They amplify it; they can't create it. The Spanish product information sets no special dose for smokers, but no tablet repairs an artery either, and going above the licensed maximum (100 mg sildenafil, 20 mg tadalafil or vardenafil) is not the answer when the effect fades.

Two things are non-negotiable. If you already carry a GTN spray or take any other nitrate for angina, none of these tablets is safe for you. And if you get chest pain during sex, stop, don't use GTN on your own after taking one of them, and call 112, saying what you took and when.

Quitting in Zaragoza: what the public system will pay for

The EAU considers it proven that lifestyle changes can improve erections in some groups of men, though nobody can promise you a timescale. Stopping smoking also does more for your heart than anything else on this page.

In Aragon, help starts at your centro de salud (health centre), with your GP or the practice nurse. The regional government's Dejar de fumar page, in Spanish, links to a 2026 list of stop-smoking services; within the city, the centros de salud San Pablo, San José Sur, Seminario, Delicias Sur and Valdespartera-Montecanal run dedicated clinics.

The conditions are strict. You need to be enrolled in a regional stop-smoking programme, smoke at least 10 cigarettes a day, score 7 or more on the Fagerström dependence test and have tried to quit in the past year. The public system pays for one attempt a year, one pack per prescription. Without a tarjeta sanitaria you can still get these medicines on a private prescription, paying the full price. No e-cigarette is authorised in Spain as a medicine for quitting.

Work out your pack-years

This is the number your doctor will write down: packs of 20 a day multiplied by the years you've smoked. Move the sliders; it's arithmetic, not a diagnosis.

Do you get chest pain or tightness walking uphill or climbing stairs?

Smokers' questions from Zaragoza

Will Viagra still work if I keep smoking?

It can, because it amplifies whatever signal your arteries can still produce. But it doesn't slow the damage, and if the effect wears off, that is a reason to see your GP again rather than to take more.

Can I get varenicline (Champix) on prescription in Zaragoza?

Yes, as long as you meet the funding conditions: enrolment in the regional programme, at least 10 cigarettes a day, a Fagerström score of 7 or more and a quit attempt in the past year. Otherwise a doctor can still prescribe it privately, and you pay the full price.

Will the public system cover my Viagra or Cialis?

No. In the Health Ministry's funding database, Viagra, Cialis and the other erection tablets are listed as not funded, so you pay for them even with a public prescription. You do need that prescription, though: Zaragoza pharmacies won't sell sildenafil, tadalafil or vardenafil without one.

Is vaping kinder to my erections than smoking?

We know of no Spanish guidance that recommends it for that purpose, and no e-cigarette is authorised in Spain as a quit-smoking medicine. The funded treatments in the table above are the ones with evidence behind them.

Other cities: heart medicines and nitrates in Las Palmas de Gran Canaria, weight, exercise and sleep apnoea in Gijón, stress and anxiety in Bilbao.
