Viagra in Seville: staying safe through a hot summer

Anyone on riociguat or a nitrate (GTN spray, isosorbide mononitrate, nitrate patches) must not take ED tablets. Heat, alcohol and blood-pressure or prostate medicines also make dizziness more likely. In Spain, prescription-only medicines may not be sold online, and customs can hold a parcel that contains them. We do not check prescriptions: talk to your doctor before you order.

Plenty of people who move to Seville, or come for the winter and end up staying into June, are caught out by the summer. The heat is not just uncomfortable; it changes how your body handles blood pressure. If you also take sildenafil (Viagra), tadalafil (Cialis), vardenafil or avanafil (Spedra), and perhaps a tablet for your prostate or your blood pressure, it is worth knowing how those things add up.

Everything below comes from the Spanish product information for these medicines (the ficha técnica and the patient leaflet, or prospecto), published by the Spanish medicines agency AEMPS in its CIMA database. The brand names are the same as in the UK and Ireland.

Small drop, bigger sum

All four tablets widen blood vessels. On their own they lower blood pressure only slightly: 100 mg of sildenafil cut it by 8.4/5.5 mmHg at most, on average, in people lying down. Most men never notice.

Heat works the same way. To shed warmth your body opens up the vessels in your skin, and sweat takes water and salt with it. Andalusia's regional government runs a heat protocol every summer (in 2026, from 15 May to 30 September), and it names people on blood-pressure medicines or diuretics, and people who drink, among those most at risk. ED tablets are not on that list, but they push in the same direction. The avanafil product information makes the link itself: low blood volume, which is what dehydration gives you, can affect how safely avanafil combines with an alpha-blocker.

Alcohol, in numbers you might recognise

The tadalafil study used about 0.7 g of alcohol per kilo of body weight, roughly 180 ml of vodka for an 80 kg man. That works out at around seven UK units, or three pints of ordinary lager. Average blood pressure did not fall further than with the alcohol alone, but some volunteers became dizzy and their blood pressure dropped when they stood up. The Cialis leaflet asks you to avoid a blood alcohol level of 0.08% or more: that is the drink-drive limit in England and Wales, and higher than Spain's, which is 0.05% for most drivers.

The other three: sildenafil 50 mg and vardenafil 20 mg did not deepen the alcohol-related drop at the levels tested (80 and 73 mg/dl). Avanafil is the exception, and its leaflet says so plainly: alcohol with avanafil can lower blood pressure, speed up your heart and make you faint. With any of them, drink works against the erection itself, as the Viagra and Spedra leaflets point out.

Medicines that often come with retirement

Spanish boxes use Spanish spellings, so here are the names to look for.

  • Alpha-blockers for the prostate or blood pressure: tamsulosina, doxazosina, alfuzosina, silodosina, terazosina. You should be stable on the alpha-blocker first. After that, the product information suggests considering 25 mg as the first dose of sildenafil, and starting vardenafil at 5 mg and avanafil at 50 mg. Tadalafil with doxazosin is not recommended at all: in the study the extra drop lasted at least 12 hours and could cause fainting.
  • Other blood-pressure tablets (amlodipino, enalapril, losartán and so on): with amlodipine, sildenafil 100 mg took off about 8/7 mmHg more. The tadalafil information says the extra fall is small if your blood pressure is well controlled and larger if it is not. Sacubitril/valsartan (Entresto) needs extra care with sildenafil.
  • Nitrates and riociguat: never with any of the four.

Tadalafil adds one more wrinkle. It keeps working for up to 36 hours, so a tablet taken on Friday evening is still active during Saturday's heat. On the 5 mg daily dose it is always there, so drinking water steadily has to become part of your summer routine.

Planning a summer evening? Check what adds up

This doesn't work out a dose. It just pulls together what the product information says about your situation.

Do you use a nitrate (GTN, isosorbide mononitrate, a nitrate patch) or riociguat?

Which tablet has your doctor prescribed?

Today or tonight…

Feeling faint after taking it

Sit or lie down somewhere cool, wait until it passes and get up slowly. Don't take a second tablet, don't carry on drinking and don't drive. If it happens every time, tell your doctor: a lower dose or a different timing may be all you need.

Call 112 (operators speak English) if you get chest pain, pass out, suddenly lose sight or hearing, or have an erection lasting 4 hours or more. With chest pain, tell the crew you took an ED tablet so you are not given a nitrate. If someone may have taken too many, ring the Spanish poisons line, Servicio de Información Toxicológica, on 91 562 04 20.

Seeing a doctor in Seville

If you are covered by the public system, through work, a pension with an S1 form or residence, you start with your family doctor (médico de familia) at the health centre (centro de salud) you're registered with. Appointments can be booked through Salud Responde, the regional health line, which also offers an interpreting service. Bring the boxes of everything you take: with this topic, the list matters more than anything else. Short-stay visitors use their EHIC or GHIC for care they need during the trip, or private insurance.

Don't expect a subsidy: none of the ED tablets is funded by the Spanish public system, according to the Health Ministry's BIFIMED database, so you pay the pharmacy price in full.

Your GP can refer you to urology. In the city, the urology and nephrology unit at Hospital Universitario Virgen del Rocío and the urology department at Hospital Universitario Virgen Macarena handle cases that tablets don't solve; Macarena's published list of services includes intracavernosal injections and penile implants.

Need a pharmacy at 2 am or on a Sunday? Seville's pharmacists' college runs a duty pharmacy finder (in Spanish) that shows which farmacia de guardia is open. Duty pharmacy or not, nobody behind the counter can sell you sildenafil or any other ED tablet without a prescription: they are prescription-only throughout Spain.

Summer questions from Seville

Is a glass of wine with dinner a problem?

Moderate drinking isn't ruled out for sildenafil, tadalafil or vardenafil. The trouble starts with a lot of alcohol on a hot day, especially if you also take an alpha-blocker. With avanafil the leaflet is stricter: alcohol and the tablet together can make you faint.

I take doxazosin. Which tablet is safer?

The tadalafil information says not to combine it with doxazosin. Sildenafil, vardenafil and avanafil can be started at a low dose once you are stable on doxazosin. Your doctor has to make that choice.

Does the heat stop it working?

There is no data on temperature in the product information. Alcohol and tiredness are the more likely culprits. Check how and when you took it with your doctor before you think about a higher dose.

Will a Spanish pharmacist explain things in English?

Many will, but there's no guarantee. Write down the names of your medicines in their Spanish form, and use Salud Responde's interpreting service when you deal with the public health system.

Our tablets, with summer in mind

  • Cialis Generic: tadalafil 5, 10, 20 and 40 mg. The longest-acting option, so heat the next day matters too. 40 mg is double the authorised maximum of 20 mg; we would not take it.
  • Cialis Original: Lilly's tadalafil, sold here only as 20 mg. With an alpha-blocker the ficha asks for caution and a start on the lowest dose, and 20 mg is the top dose.
  • Viagra Generic: sildenafil tablets in 25, 50 or 100 mg. The 25 mg strength is the starting dose suggested for men on alpha-blockers.
  • Levitra Generic: vardenafil 10 and 20 mg. Alongside an alpha-blocker vardenafil starts at 5 mg, which this shop doesn't stock.

One practical point: there's no Seville branch of this site, and no shop counter to buy from. Everything goes out by post or courier, and the details are on our delivery conditions page rather than here. Customs can stop a parcel, and nobody on our side asks for a prescription.

Other cities: bringing medicines and prescriptions from home in Málaga, heart conditions and nitrates in Las Palmas de Gran Canaria, side effects in Jerez de la Frontera.
